CURRENT OPPORTUNITIES

 

Chronic Care/ Telemedicine Coordinator –  Full Time

Details:

• Coaches patients/families toward successful self-management of their chronic disease.
• Facilitate patient access to appropriate medical and specialty providers as well as other care coordination team support specialists (e.g. Diabetes Educator).
• Serve as a point of contact, advocate, and informational resource for patient, family, care team, payers, and community resources and facilitate and attend meetings between patients, families, care team, payers, and community resources, as needed.
• Ensure effective tracking of test results, medication management, and adherence to follow-up appointments.
• Perform regular calls to provide patient and family support and education.
• Collect data and write brief reports to meet evaluation needs of the program.
• Assist in coordinating patients’ appointments, complete necessary intake information, set up patients on the teleconferencing equipment, when indicated sit with patients during the tele-mental health session, ensure that the teleconferencing equipment is in good working order and provide trouble shooting for system malfunctions.
• Assist physicians with the tele-psychiatry clinics and perform relevant job duties as assigned by immediate supervisor.
• To perform specified nursing functions, i.e. administration of injectable medication, perform venipunctures in accordance with nurse practice activities and organizations policies.
• To serve as case manager for assigned individuals and provide general supportive counseling as needed.
• Coordinate services and places with collateral agencies and providers, (i.e. local pharmacies, physicians, for patients receiving services through the tele-psychiatry clinic.)

Requirements:
• Current RN license in Virginia (required)
• Current BLS card (required, but may obtain upon hire)
• Previous experience in caring for chronic disease patients (required)
• 3-5 years’ experience in clinical or community health settings (preferred)
• Previous Telemedicine and/or IT experience (preferred)
• Previous Care Coordination, Case Management or Home Health (preferred)

 

 

Nurse Practitioner –  Full Time

Details:
• BCH has multiple clinic locations, NP will be scheduled to work amongst the clinics as needed.
• Ability to perform health assessments, including a thorough history and physical examination, ordering or performing certain diagnostic tests, evaluation of the psychosocial and family aspects of the situation sufficient to make a general health assessment, and diagnose the nature of common acute and chronic conditions.
• Promotes maintenance of family health, including planning or periodic health assessments and screening for early case findings, assessment and guidance with regard to health hazards, child rearing, good family health habits, family planning, mental and emotional problems and continued maintenance of contact and rapport with the family.
• Ability to manage common acute and chronic illness, including minor trauma, episodic problems, common chronic disorders, as well as counseling and education with regard to such problems, performance of minor surgical procedures and initiation of life-saving procedures in emergencies.
• Recognizes need for referral of complex and high priority cases to the physician on duty with regard complicated diagnostic problems, serious illness, complicated therapeutic problems, reevaluation of chronic conditions, etc.
• Ability to work as a member of the healthcare team, and assisting in education of patients and the target community in preventive health care and health maintenance concepts.
• Ability to relate well with patients and other healthcare personnel.
• Demonstrates willingness to educate patients and community in health maintenance and the role of the healthcare team.
• Accurate and timely completion of all clinical and professional records and documentation to assure compliance with local, state and federal regulatory, licensure and accreditation requirements

Requirements:
• Current Virginia License to practice as nurse practitioner

RN Nurse Supervisor-Med Surg/ED –  Night Shift –  Full Time
Details:

• Supervises nursing personnel in their daily work
• Maintains communication daily amongst all nursing areas and with DON
• Assist in the development, review and revision of policies and procedures that govern the Nursing Services in collaboration with DON
• Represents Nursing Services on various hospital committees
• Assists with continuing education and in-service programs are available as needed
• Assists with facilitation of Nursing Staff meetings on a regular basis
• Assigns and assists with scheduling staff
• Onboards and trains new employees
• Demonstrates knowledge and skill in ED and Med. Surg
• Demonstrates and enforces the standards of behavior

Requirements:
• Proficiency in computers (required)
• Current Registered Nurse license in the State of Virginia (required)

BLS Transportation Provider – PRN

Details:

The BLS Provider is responsible for the safe transportation of non-ambulatory patients to and from medical facilities. It is the responsibility of the BLS Provider to see that pick-ups and drop-offs are made in a proper, courteous and timely manner. The BLS Provider may be involved in all types of medical transportation.

Requirements:

• High school graduate or equivalent (required)
• Current VA Emergency Medical Technician (EMT) or higher (required)
• Current BLS certification (required)
• Current EVOC certification (required)
• Clean driving record from DMV (required)
• Minimum age 21 (required)
• Previous transportation and/or Emergency Room experience (preferred)

 

ALS Transportation Provider –PRN

Details:

The ALS Provider is responsible for the safe transportation of non-ambulatory patients to and from medical facilities. C.N.A. (Med Surg)- PRN

Duties include:

  • Inpatient-Assist patients with activities of daily living, provides basic nursing care to non-acutely ill patients and assists in maintenance of a safe and clean environment
  • Emergency Department-Assist nurses and physicians with providing care to acutely ill patients
  • Register patients and collect copays
  • Collect samples for lab analysis, gather patient information, monitoring EKGs and heart rhythm, treat wounds and assist doctors and nurses during procedures

Requirements:

  • Previous acute care hospital work experience (preferred)
  • ACLS and PALS Certification (preferred)
  • Certified Nursing Assistant (required)

 

 

 

R.N. (ER/Med Surg) – PRN

Operating Room Registered Nurse – Part Time

Details:
• Provides patient care in the Ambulatory OR setting directed toward meeting both the psychosocial and physical needs of the patient.
• Is responsible for the Central Sterile processing to ensure appropriate cleaning, disinfection, or sterilization of instruments and equipment according to manufacturer recommendations.
• Acts as a resource person and teacher and a liaison between surgeons, anesthesia providers, and nursing staff.
• Care given reflects initiative and responsibility indicative of professional expectations.
• Provides quality measure reporting to Director of Quality.

Requirements:
