As part of Bath Community Hospital’s commitment to the community, BCH conducts a Community Health Needs Assessment (CHNA) every 3 years. This process helps us determine where the unmet needs are in our area and how we, as a community, can work together to ensure our community has access to high-quality healthcare and wellness programs.

Our first CHNA was conducted in 2012. Based upon that assessment, our focus included

Access
Nutrition, Exercise, and Obesity
Chronic Diseases
Mental Health
Cost/Affordability
During 2015, we are worked diligently to gather data and met with community representatives to determine our largest healthcare gaps in services. To review the findings, please click here.  Areas identified include

Substance Abuse
Exercise, Prevention & Education
Mental Health
Aging Population & Chronic Disease Management

Our Implementation Strategy to address these needs can be found here.
If you have questions or comments about the 2015 CHNA contact Pat Foutz at 540-839-7055 or Amy Phillips at 540-839-7179.
